Insurance

We are not accepting new HMO, EPO, Medicare or Texas Medicaid patients at this time. We are accepting PPO patients through our cash pay model. We will be discontinuing our insurance contracts as of Jan. 1, 2024. After Jan. 2024 you will be able to see us for the cash pay rate. Insurances with “Out of Network” benefits may be able to be reimbursed for some or all of the cost of their visit. Our cash pay rate is $60-$115/visit. As of Jan. the cost will be $60 for a level 3 visit (UTI/sinus infection/uncomplicated injury) or $115 for level 4 visit (wellness/chronic medical issues/ADD/complicated injury). Additional cost for procedures may apply. All labs, imaging and PT will still go through insurance.


before your appointment

  • New patient forms will be available on our portal. A link to create a profile is sent to the email you provide when you make your appointment. You will use your last name and date of birth only to create your profile. If you have any issues with filling out the forms please arrive 15 minutes early to fill out forms in the clinic.

  • Make sure to bring a photo ID and your insurance card(s).

  • Minors must be accompanied by a parent or guardian unless previous arrangements have been made.

  • We will need to know your current medications. You can bring your pill bottles with you or have the pharmacy print a list of current medications for you.


Payments

  • We accept cash or credit cards (Visa, Mastercard, Discover, American Express)

  • Co-payments are expected in full at time of service.

  • We do not accept Care Card at this time.

  • We do not currently have online payments available. Please call, mail, or stop by the office to make payments on your balance.


workers compensation

We do accept workers compensation. Please contact us directly to see if we accept your plan.


Phone calls

  • As a small office we are often busy with patient care. Please leave a message and we will make sure to return your call in a timely manner.

  • Messages left after 4pm M-Th or after 11:30pm on F may be returned the next business day.

  • Calling multiple times interferes with us being able to return patients calls. Please respect that we need time to care for other patients and we will make sure that you also receive the same respect.


Results

  • Once your labs are drawn or test(s) have been performed we typically receive your results anywhere from 2-5 business days after your test. Please allow us time to receive and review your results before we contact you.

  • If your test was ordered STAT we should notify you of results within 48 hours. If something emergent is found upon review of your studies we will notify you as soon as possible to make a follow-up appointment.

  • If you have a follow-up visit already scheduled please keep that appointment so that you may review your findings with the physician, we will still call you and inform you of your results.

  • If you do not hear from our office within 3 days of having your study, please give us a call.

  • In order to expedite your results make sure that we have your most current telephone number and address.


Medication/Refills

  • If you would like a prescription refilled please make your request online through the portal or ask your pharmacy to fax the refill request to our office at 866-358-3313.

  • We ask that you give us 3 days notice on prescriptions refill requests.

  • If you would like to contact our office for your refill please be aware that we may not be able to return your prescription refill call until the end of the business day.

  • If you are on medications for chronic conditions we may ask that you be seen prior to giving further refills.

  • We will receive refill requests from 8:00 A.M. – 5:00 P.M. Monday thru Friday. Any attempt made on the weekend will not be reviewed until the next business day. Please avoid waiting until Friday afternoon to request a refill. Call your pharmacist during business hours no later than Thursday afternoon and ask them to please fax your request in so that we may have ample time to respond.

  • As of Jan. 1, 2019 we will not prescribe opioid medications for longer than 6 weeks. If you require these control medications longer than 2 weeks you will be referred to pain management and have 30 days to transfer opioid prescribing to that provider. Our goal is to find the source of your pain and help you get to a point where you do not require medication. We are happy to continue to help you to heal through care other than controlled medications.
